PRIA: robotic parotid surgery without a face or neck scar
Parotidectomy by Robotic Infraclavicular Approach. Tumours of the parotid salivary gland are removed through small cuts below the collarbone instead of a cut in front of the ear and down the neck.
For patients
What is the parotid gland?
The parotid is the largest salivary gland. It sits in front of and below the ear. A lump here is the most common sign of a parotid tumour.
The usual operation, and the difference with PRIA
A conventional parotidectomy is done through a cut that runs in front of the ear and down into the neck. PRIA uses the same route below the collarbone as RIA-MIND:
- Small cuts are made below the collarbone.
- The robotic instruments travel under the skin up to the parotid gland.
- The tumour is removed while the facial nerve is protected.
- Clothing covers the scars, so none are visible on the face or neck.
What to expect
- The operation is done under general anaesthesia.
- A small drain is usually placed for a short time.
- Your face may feel numb or slightly weak for a while. This is checked at every follow-up visit.
For referring doctors
Technique
Four ports are placed in the infraclavicular region and the da Vinci Xi system is docked. A subplatysmal flap is raised to reach the parotid. The facial nerve is identified and preserved, and both superficial and deep lobe tumours can be addressed. A drain is placed at the end of the procedure.
Referring a patient
Please share the FNAC or biopsy report and the imaging (MRI preferred for deep lobe lesions). See information for doctors.
Common questions
Are parotid tumours usually cancer?
Most parotid tumours, roughly 8 in 10, are benign (not cancer). They are still usually removed, because they keep growing and some can turn into cancer over time.
What about the facial nerve?
The facial nerve, which moves the muscles of the face, runs through the parotid gland. The surgeon finds and protects it throughout the operation. Temporary weakness of part of the face can happen after any parotid surgery and usually recovers.
Is PRIA suitable for every parotid tumour?
No. Suitability depends on the size and position of the tumour and on whether it is benign or malignant. Dr. Suraga will advise after examining you and reviewing your scans.
